**New Delhi: **A preliminary investigation by the Aircraft Accident Investigation Bureau into an incident involving an Air India flight from Phuket to Delhi has found that a sudden failure of the aircraft’s hydraulic systems contributed to a significant loss of altitude during the flight. The preliminary report has also raised concerns over the conduct of the pilot who was operating the aircraft at the time of the incident.

> The Airbus A320neo was flying at an altitude of around 36,000 feet when all three of its hydraulic systems reportedly became inoperative within a matter of seconds. According to the investigation, the sudden loss of hydraulic functionality resulted in an “altitude upset”, a condition involving an unexpected deviation in an aircraft’s altitude or flight attitude. There were 137 passengers on board the aircraft at the time.

The report states that the pilot in command did not directly handle the primary flight controls during the incident. It also notes that the results of two tests conducted on the co-pilot were negative.
